Jury-rigging your equipment is seldom a good idea. And the recent
death of a forklift operator has prompted OSHA to issue a safety
warning against the unauthorized modification of industrial trucks.
A worker at a Chicago-area manufatcturer was killed when an 8-
foot, 25O-pound extension boom and a 500-pound forklift carriage
collapsed on him. The boom had been attached to the top of
the forklift carriage and used to lift the end of 5-ton silo.
The worker was under the boom measuring the diameter of the silo
when the carriage collapsed.
An OSHA hazard-information bulletin issued June 20 reminds
workers and managers that modification of industrial trucks,
without the written approval of the manufacturer, is a violation
of safety standards. In addition, workers are prohibited
from placing themselves under a forklift carriage.
